Veterinary Consent or Referral

  1. Fully read our terms and conditions. These are included on the Veterinary Consent/Referral Form. The form can be posted to you or if you click on the link below you will be able to download it.
  2. Complete the Veterinary Consent/Referral Form, ensuring that you get your vet to complete and sign the relevant section.
  3. Post or scan and email the completed form back to us. If you are passing by us you are also welcome to deliver the form in person.
  4. Once we have the above mentioned form we will then speak with your vet if required. We will then contact you to arrange an appointment for your first session.

We offer daytime, evening and weekend appointments.

Payment is required on the day of treatment and can be made by cash or card.
